Epworth Sleepiness Scale Formula

The Epworth Sleepiness Scale (ESS) score is the sum of eight ratings, one for each everyday situation. You rate each situation from 0 to 3, so the total runs from 0 to 24.

ESS = I1 + I2 + I3 + I4 + I5 + I6 + I7 + I8

  • I6 = chance of dozing while sitting and talking to someone (0 to 3)
  • I7 = chance of dozing while sitting quietly after lunch without alcohol (0 to 3)
  • I8 = chance of dozing in a car while stopped for a few minutes in traffic (0 to 3)

Every item uses the same rating scale: 0 means you would never doze, 1 a slight chance, 2 a moderate chance, and 3 a high chance of dozing. Rate the chance of actually falling asleep, not just feeling tired, based on your usual recent weeks. If you have not been in a situation lately, estimate how it would affect you.

The calculator adds your eight answers to produce the 0 to 24 total, then matches that total to a severity band. If a provider has already given you a total, the interpret mode maps that number to the same bands without re-entering every answer. It also splits your total into two groups so you can see where the sleepiness comes from:

Active subtotal = I6 + I8
  • Active subtotal = chance of dozing during the two more alert situations, talking and sitting in stopped traffic (0 to 6)
  • Passive subtotal = the remaining six restful situations (0 to 18)

A high active subtotal is worth noting, because dozing while talking or while stopped in traffic is less usual than dozing while resting and is one detail a provider may ask about.

Epworth Score Ranges and Typical Values

The first table shows how the total maps to a severity band. The second table lists average ESS scores reported for different groups in the original 1991 validation study by Murray Johns, so you can see roughly where a score tends to fall for each group. The condition averages are the piece most score pages leave out, and they give a real reference point instead of a single normal or abnormal label.

These averages overlap, so a single score cannot tell you which condition, if any, you have. They show the pattern: scores near 6 are typical without a sleep disorder, scores near 12 are common with sleep apnea, and scores near 17 or higher are seen with narcolepsy and idiopathic hypersomnia.

Example Problems

Example 1. You rate the eight situations as 1, 2, 1, 2, 3, 0, 1, 2. Adding them gives 1 + 2 + 1 + 2 + 3 + 0 + 1 + 2 = 12, which falls in the 11 to 12 band, mild excessive daytime sleepiness. The active subtotal is item 6 plus item 8, or 0 + 2 = 2, so the calculator flags that you reported a chance of dozing while stopped in traffic, which is worth mentioning to a provider.

Example 2. You rate the situations as 1, 1, 0, 1, 1, 0, 1, 0. The total is 1 + 1 + 0 + 1 + 1 + 0 + 1 + 0 = 5, which lands in the 0 to 5 band, lower normal daytime sleepiness. The active subtotal is 0, so no alert-situation flag appears.

What is a normal Epworth Sleepiness Scale score?

A total of 0 to 10 is considered normal daytime sleepiness. Scores of 0 to 5 sit at the lower end and 6 to 10 at the higher end of normal. A total of 11 or more points to excessive daytime sleepiness that is worth discussing with a healthcare provider.

Does a high score mean I have sleep apnea or narcolepsy?

No. The ESS measures the symptom of daytime sleepiness, not any specific condition. A high score means you tend to doze off easily, which can happen with sleep apnea, narcolepsy, insufficient sleep, medications, and other causes. It is used as a screening step, and a provider confirms a diagnosis with a sleep study or other tests.

How should I answer if I have not been in one of the situations recently?

Estimate how the situation would affect you if it happened now. The scale asks about your usual way of life in recent times, so a reasonable estimate for a situation you rarely encounter is expected and keeps the total meaningful.
