Braden Score Formula

The following formula is used to calculate the Braden Score:

BS = Sensory+Moisture + Activity + Mobility + Nutrition + Friction

Variables:

  • Mobility (1-4 points)
  • Nutrition (1-4 points)
  • Friction & Shear (1-3 points)
