Timed therapy codes are commonly billed in units, with each unit representing a fixed block of time. Many PT, OT, and SLP services use 15-minute timed units.
- Standard 15-minute units: 30 minutes = 2.00 units
- Standard 15-minute units: 45 minutes = 3.00 units
- Medicare 8-minute rule: 20 minutes = 1 unit

Therapy Minutes to Units Formula
Arithmetic modes divide minutes by 15, 10, or 30 and report time-unit equivalents. These mathematical equivalents are not, by themselves, billable units.
For the Medicare 8-minute-rule estimate, applicable one-on-one 15-minute timed-code minutes for the same patient and date are totaled. The whole-unit boundaries include: 0–7 minutes = 0 units, 8–22 = 1, 23–37 = 2, 38–52 = 3, and 53–67 = 4.
Medicare Timed-Code Scope
The calculator estimates only the total number of timed units. The units must still be allocated across the individual timed codes according to the minutes for each service and current Medicare instructions. Untimed, group, and concurrent minutes are excluded from this estimator.
Billing Caution
Group, concurrent, supervised, untimed, and payer-specific services can follow different rules. Medicare policies and other payer contracts may differ, so verify current requirements, documentation, medical necessity, and code-level allocation before submitting a claim.
